PDF form for complete termination of the line
Do you want to complete the complete termination of your telephone number and would like to understand how to find the appropriate PDF form to download, fill in and send to the manager? Then do this: click here to connect to the service MyFastPage with your credentials. If you don't remember them, check the SMS that was sent to you by Fastweb when you subscribed with the manager or click on the link Did you forget your password? attached at the bottom of the page that has opened and request them again following the instructions on the screen.
Once logged in, fill out the digital form that is proposed to you on the screen by typing your personal data (name, surname, email, etc.) in the appropriate text fields, presses the button Send which is located at the bottom and wait a few moments for the page with the PDF form to be downloaded to be loaded to terminate the contract.
So save the form on your computer and print it. If you don't know how to download the PDF on your PC, click on the icon arrow or on that of floppy disk, select the item Save object as/Save link as from the menu you see appear and the download will start immediately.
Then fill out the form obtained in all its parts indicating your personal data and check the items relating to the services that you intend to deactivate (eg. voice and Internet connectivity). Once this is done, send the form by registered letter with return receipt at: FASTWEB SpA - P.O. Box 126 - 20092 Cinisello Balsamo (MI).
Please note that once the form is submitted it will take approx 30 days to cancel the contract. During this period you will be able to continue to use the services included in your subscription without problems and you will also have to continue to pay the fee regularly.
Also keep in mind the fact that once you have sent the form for the cancellation of the line to Fastweb you will also have to return to the operator the modem and any other equipment that the latter had granted you on loan for use in order to avoid incurring salary penalties. You will also have to pay the operator's decommissioning costs and any additional surcharges in case of breach of contract. PDF form for withdrawal within 14 days
As anticipated at the beginning, the other case in which it is necessary to download the PDF form for cancellation and send it to Fastweb is the early withdrawal. More precisely, if you have signed a contract with Fastweb for less than 14 days and if the signing took place outside the company's business premises (eg via call-center) you can take advantage of the right of withdrawal and cancel your subscription without incurring additional costs and without particular waste of time.
To find the PDF form suitable for the purpose, click here to connect to the Fastweb website, then presses on the item Fixed offer rethinking form (early withdrawal form), download the form on your computer by clicking on the icon arrow or on that of floppy disk and select the item Save object as/Save link as then print the document. Then you will have to fill it in and always send it by registered A / R to the address FASTWEB SpA - P.O. Box 126 - 20092 Cinisello Balsamo (MI).
As an alternative to sending the form, you can take advantage of the right of withdrawal by contacting Fastweb customer service (if you remember correctly, I explained how to do it in my article on how to talk to Fastweb operator) and communicate to the operator with whom you will find yourself talking about what your needs are.
Switch to another operator
As anticipated in the previous lines, as regards the switching to another operator there is no need to download and send any PDF form but I still want to show you the procedure to follow in order to provide you with a complete overview of what to do. More precisely, all you need to do is contact the new operator with whom you intend to subscribe, notify the latter of the migration code Fastweb and follow the instructions that will be provided by the new provider.
The migration code, in case you don't know what it was, is nothing more than an alphanumeric code, consisting of 7-18 characters, which allows you to uniquely identify the telephone lines and authorize the passage from one provider to another (in this way the cancellation of the old contract and the activation of the new subscription are managed directly by the providers and avoid the user annoying waste of time between various forms, offices and repeated contacts with customer service). You can find the migration code on your bill or by logging into MyFastPage.
Switching from one manager to another usually takes about 10 days. In order for the procedure to be effectively and quickly successful, it is necessary to sign some documents that are received at home and that must then be sent by fax or registered letter according to the methods indicated by the provider chosen for the passage. In all cases, a copy of your identity document must be attached. As for the costs of decommissioning and returning the equipment, I invite you to consult the following section specifically dedicated to the issue.
Costs and return of the equipment
Let's now review what i are deactivation costs that must be supported with Fastweb after sending the PDF form for cancellation (and not only). For those who terminate their subscription without switching to another operator, the price to be paid is equal to 70,60 € if you have appliances on loan for use / rental, while it is equal to 62,60 € if you have your own devices. On the other hand, those who migrate to another operator or return to TIM pay 42,60 € if you use appliances on loan for use / rental or 34 euros if you use personal devices.
As for the return of equipment, this must be done by 45 days from the cancellation of the contract with Fastweb. In the event that the established deadlines are not respected, the payment of rather hefty penalties ranging from a minimum of 10 € a 110 € for each device. Below you will find the exact amount of the penalties relating to the non-return of all the various equipment rented from Fastweb.

Cancellation of mobile line
As I have already anticipated at the beginning of the guide, as regards the mobile line to make the cancellation it is not necessary to download any PDF form but for completeness of information I will provide, even in this case, to provide you with the necessary explanations.
completely cease your use, you must send a registered letter with return receipt with the written cancellation request and the related reasons to the address FASTWEB SpA - P.O. Box 126 - 20092 Cinisello Balsamo (MI). In the request, remember to indicate your full name, your tax code, your customer code and your telephone number and to sign everything. Also remember to attach a photocopy of your valid identity document.
When Fastweb receives the registered letter, the operator's customer service will contact you to ask for confirmation of the documentation received, after which it will proceed by going to deactivate your user account.
If you want to do the number portability (together with any residual credit) you do not have to do absolutely anything except start all the procedures for the transfer with the new manager. The operation does not involve extra costs except in the case in which the contract currently in place with Fastweb provides for time constraints and then it is necessary to reimburse any bonuses and promotions used. The same applies if the subscription also included the purchase of a smartphone or tablet: in the case of potability, the remaining installments must be paid to buy the device.
